| Attendees in this session will participate in a live RCA team meeting on an actual failure using real failure data. Team members will be provided initial facts about an unexpected failure, then evidence will be provided based on their Failure Scene Investigation (FSI) data collection efforts as it is uncovered. Participants will develop hyp o theses about how the failure modes could occur and use collected evidence to continually drill down the cause-and-effect chain until they uncover the physical, human and latent root causes. |
